Date: Not dated, but based on the phone number, probably from the 1950s or early 1960s.
Condition: Good, I'd say. The matchbook has some light staining and slight edge wear. The striker strip looks like it might have been used once (or perhaps it's just a light scratch). The matches have been removed. (Please see scans.)
